2018: The remote patient monitoring (RPM) service is recognized by CMS. CMS allows providers to get reimbursement for RPM services. AI and wearable technology have enabled higher adaptation and better utilization of RPM.
2019: The medicare physician fee schedule was released by CMS in November 2019, and it had new codes for billing for remote patient monitoring. Some new CPT codes became active from jan 1, 2019.
2020: The medicare physician fee schedule for this year introduced additional CPT codes. Provider, while spending more than 20 minutes on remote patient monitoring could charge for the service.
2021: This year led to more refinement from the Medicare Physician Fee Schedule. CMS provided an update on how providers could bill for RPM services, including the eligibility criteria, reimbursement timelines, and associated conditions.

CPT Code 99454
It reimburses providers for the monthly supply and setup of remote patient monitoring (RPM) devices, along with the collection, transmission, and monitoring of patient-generated physiological data over at least 16 days within a 30-day period.
CPT Code 99457
Bills for the first 20 minutes of clinical staff time spent on remote patient monitoring, including interactive communication with the patient or caregiver during a 30-day period.
CPT Code 99458
Bills for each additional 20 minutes of clinical staff time spent on remote patient monitoring, care management, and patient education within the same 30-day period.

The code allows for billing of each additional 10 minutes of interactive communication between clinical staff and the patient, beyond the initial time covered under CPT Code 99457.
